Client recommendations are based on the principles of human clinical nutrition and Functional Medicine to help you with your health goals. Each client will receive personalized care and recommendations. The ultimate goal is that each client will learn and adopt lifestyle changes, including diet, exercise and stress management protocols to lead a healthier life. Consultations are held in person, via a HIPAA compliant online platform or by phone with Sasha Campbell.

INITIAL CONSULTATION | 2 hours

The initial consult will be comprised of 2 separate appointments spaced 1 week apart. 

1. Assessment Appointment (1 hour)

  • Sasha will complete an initial assessment prior to your scheduled appointment, based on the online intake forms and medical records that are provided by you, the client.
  • Any items that need clarification will be reviewed during the first appointment in order for Sasha to have all of the details she needs for a customized plan.

2. Review and Plan Appointment (1 hour)

  • Your personalized Functional Medicine Nutrition Prescription will be reviewed and include a summary of findings, recommendations for nutrition, exercise, sleep/restoration, supplements and goal setting.
  • All necessary supporting materials will be sent to you (i.e. recipes, dietary plans, etc.). 
  • Additional tests/labs may be requested.
  • You will walk out of this appointment ready and equipped to embark on your health journey.

FOLLOW-UP CONSULTATION | 30 minutes

Follow-up consultations are proven critical to the success of a nutrition therapy plan. Developing new eating and lifestyle habits takes time, motivation, and coaching to allow these changes to stay with you for a lifetime. With regular follow-ups, we are also able to make necessary adjustments to your plan that may be required to offer the most support to you on your health journey. 

RE-EVALUATION CONSULTATION

These are recommended at 3 months, 6 months, and 12 months and for clients who have not seen Sasha for over 6 months.

CANCELLATIONS

Initial consultation appointments require an upfront deposit of $120 at time of booking. This deposit is forfeited if an appointment is canceled or rescheduled less than 24 hours in advance of the appointment. If the appointment is re-scheduled within 24 hours of the appointment, it may still be applied toward the rescheduled initial consult. 

All follow-up consultations must be canceled or rescheduled no less than 24 hours in advance notice to avoid full appointment charge. 

PAYMENT OPTIONS/POLICIES

We accept checks and all major credit cards (Visa, Mastercard, Discover and American Express) for all services offered and supplements. Some HSA/FSA cards may may be accepted for services only. It is the client's responsibility to determine if the HSA/FSA will cover nutritional consultations. 

Securing your initial appointment:

Option 1: Purchase the package that best fits your goals and needs, paying in full.

Option 2: A deposit of $120 secures your scheduled initial visit and is due at booking. The balance amount for the Initial Consultation of $120 is due no less than 72 hours in advance of your first appointment. If you have not already paid in full at booking, a payment link will be emailed to you one week prior to your appointment as a reminder of your balance due. 

If the package is not purchased or the deposit is not received within this 72 hours (3 days) prior to your scheduled appointment, your appointment will be cancelled and must be rescheduled. 

REFUNDS

  • Pre-paid plans and packages: Refunds will not be given for pre-paid plans and packages
  • Refunds will not be provided for the deposit that secures your initial appointment time slot. 

INSURANCE INFORMATION

We do not accept any form of health insurance for consultations or supplement purchases. You will be provided with invoices that can be submitted to your insurance company. It is your responsibility to check with your insurance carrier to determine reimbursement eligibility. Sasha Campbell will not file claims on your behalf, nor will she assist with any claim resolutions or disputes. ICD10 codes and CPT codes will not be provided. If you use a HSA/FSA account for your payment, the same policies apply.

MEDICAL RECORDS

In compliance with HIPAA, medical records can only be released with your written consent. You are responsible for requesting and obtaining all necessary medical records for your appointment. A release form is provided for you to send to your medical providers. Your records should be mailed to Sasha Campbell at Vital Fx Nutrition one (1) week prior to your scheduled appointment.

SUPPLEMENTS

In determining what you need to restore health, our office only suggests high-quality professional supplements. You are not under obligation to purchase these supplements directly from our office. If they are suggested as part of your plan, it is important to use them and follow the directions provided regarding dosing, regardless of where you purchase them. In choosing supplements, we consider:

  • The quality of science behind the product
  • The quality of the ingredients
  • The quality of the manufacturing process
  • The synergy among the ingredients

We cannot guarantee the same quality or expected effectiveness of supplements purchased through health food stores, retail stores or through online suppliers.

Individuals participating in a pre-paid plan will be provided with a courtesy discount on all supplements. 

TERMINATION OF SERVICES

This is rare, and will be avoided whenever possible, however, Vital Fx Nutrition does have the right to terminate services with a 30-day written notice. If services are terminated, a refund will be given for the remaining visits owed. Refunds will be made within 30 days of notification. As the client, you have the right to terminate services at any time and agree to give proper notification (see cancellation policy). 

PREPARING FOR YOUR CONSULATATION

You will be added as a client to our secure, HIPAA compliant system. You will be emailed forms to complete. You may either stamp and mail the forms back to the office or scan and email via our HIPAA compliant Vital Fx Nutrition G Suite. Camscan is an easy to use smartphone app for scanning and emailing documents.  Please note that these initial forms will take some time to complete. The more complex health history you have, the longer it can take to complete the forms. Accuracy and details when completing these intake forms will allow for the maximum use of time during your consultation. 

You will be asked to complete a 3-day diet diary. It is important to not make any changes to your current diet during this time so that we can have an accurate picture of your eating habits.

For telehealth clients, we will either schedule a phone call or you will be sent a link 24-hours in advance of our scheduled appointment. If you are local to the Rapid City office, your appointments with Sasha Campbell may be conducted in person.
